At Mercy Plus, we approach all patients as individuals, carefully assessing the client’s physical, financial, social and psychological needs. We will work closely with caregivers and physicians to develop a personalized plan of care and assign a professional that best matches the needs of the client.
The plan of care will outline the services, frequency and duration of visits, and will coordinate with our internal and external health providers as necessary. After a caregiver is assigned, our agency performs routine follow-up visits at your home to make sure all expectations are being met.

Our home health services include:

Companion Care
Companion Care provides emotional support and companionship for seniors who are generally healthy and want to remain independent in their home. It can also include a range of non-medical services, including basic cooking, cleaning and support that help make a senior’s life more manageable.

Medication Management
Medications prescribed by your physician, when taken correctly, support the healing process. Knowledge and education about medications are key to maximizing their safety and effectiveness. Our home care team can oversee the administration of prescribed medications, educated the patient and family on proper dosages, frequency and side effects, evaluate therapeutic responses to medications, assess for duplicate medications or drug interactions, and assist in developing a medication schedule to enhance compliance.

Pediatric Nursing
Our professionals specialize in caring for children of all ages, from newborn to adolescents.  All of our nurses are well-trained and up-to-date with pediatric assessment tools, family life dynamics, and environmental concerns. They are also efficiently skilled at delivering rehabilitation therapies for patients with mild to severe developmental needs in concert with both family members and school administrators.

Our pediatric services include long-term skilled nursing for medically complex and technology dependent children, rehabilitation and developmental therapies, ventilator therapies, and support for families with newborns.

Personal Home Care
Our compassionate, trustworthy and certified Home Health Aides and Personal Care Attendants are available to assist with dressing, eating, grooming, medication reminders and light housekeeping. They are trained in all areas of personal care needs, are CPR certified and have met all requirements for employment, including background checks. They work hand in hand with our nurses and therapists to ensure that each patient’s care plan is carried out according to the wishes of the patient, their family and the patient’s physician. Our personal home care team is here to give you peace of mind, knowing that your loved one is in good hands.

Private Duty Nursing
Our caring, professional staff offers you or your loved one customized assistance and hands-on care, twenty-four hours a day. We provide a full spectrum of private care in the client’s private home, assisted or long term care facilities, rehab centers and hospitals.

Our staff will also assist with the personal care services needed to maintain independence in the home, including meal preparation, personal grooming, laundry, light housekeeping, medication reminders, companionship and caring for bed-bound clients. When one-on-one care is needed, we will be there for you, whether it’s an hour a day or for overnight stays.

Skilled Nursing Care
Mercy Plus Home Healthcare provides highly skilled, compassionate nurses on a full, part-time or per visit basis, who work closely with the client, their physician, family members and care providers to maintain the client’s health, and prevent further illness and injury. Our nurses conduct patient and environment assessments, and coordinate care with all team members. They provide treatment and educate patients and other caregivers on the disease or injury process, treatment plan, medications, diet and nutrition, foot care, catheter and wound care.
